Our plan at Anthem does a good job at sharing our cost and managing the provider network. In this case, I did not have to pay out of pocket since my deductible had already been satisfied for the year. Anthem reduced the original billing which was over 3 x higher than the final billed paid of $800.

I'm on a PPO 1000 plan, with a $1000 deductible and 100% coverage on preventative colonoscopies. This is my 3rd colonoscopy since turning 50 and the previous two were covered 100% as preventative. This was deemed to be diagnostic by UHC and therefore not covered at 100%, even though the exact same procedure code was used as the previous one in 2014. They tell me that since polyps were found 5 years ago, that this one is not considered preventative. I've appealed and lost and am unable to find anyone to help me dispute this.
